Death Certificates

To request a death certificate by mail using our application:

2)	Fill out the application and send it to the address listed below.

To request a death certificate by mail, write a letter including the following information:

• Relationship.     

Members of the decedent's immediate family may obtain a certified copies of death certificates. Relationship must be stated, i.e.,

(I wish to obtain a copy of my Grandfather's death certificate.)

•  Reason the certificate is needed.
  

•  Full name of person named on the record.
  

•  Date of death.
  

•  City and county of death.

Be sure to sign the letter.

The fee is $20 and additional copies of the same record issued at the same time are $3. Please include a business sized self-addressed, stamped envelope. Do not send cash. We accept checks or money orders made payable to: VITAL STATISTICS and mail to:

Mail requests are processed in 24 to 72 hours after receipt of your request.
