Passport Information

From the Office of Joseph J. Czarnezki
Milwaukee County Clerk

A reminder to all Milwaukee County citizens -- the County Clerk's Office is now an acceptance agent for U.S. Passports. Passport photos are also available at a cost of $10.00. We will be taking passport applications during our regular business hours of 9:00 a.m. to 3:30 p.m. All persons applying for a passport, including children, must be present during the application process.

The cost of a passport for adults (persons 16 years of age and older) is $100.00. You will need two checks -- one for $75.00 made out to the U.S. Dept. of State and the second made out to the County Clerk for $25.00. The cost of a passport for a child under the age of 16 is $85.00 (one check for the U.S. Dept. of State for $60.00 and one to the County Clerk for $25.00). Both parents must sign the application of a passport for a child under the age of 16.

There is also an expedited service available for an additional fee of $60.00 for each passport, plus the cost of overnight mail. You can expect your passport in three weeks.

We will need you to bring in a certified birth certificate, a valid ID (driver's license) and married women will need to bring in a marriage certificate if they have changed their last name.

Passport cards are also available. You will need the same documents as with a passport. The fees for passport cards are $45.00 for Adults and $35.00 for children under age 16.

For travel details, please see the U.S. State Department website at http://travel.state.gov/passport/forms/forms_847.html. The passport application form can also be downloaded from this website.
